New revised claim form for 2010 has been released. Please DO NOT use the old forms. Please obtain prior approval from your department elder for the expenses. Claim forms will be processed when all original receipts with complete information are attached.
最新修訂的二零一零年「索款表格」（Claim Form）已準備好。 各項開支須事先得部門長老批核， 填妥的「索款表格」須連同收據正本方可接受辦理。
